From the very early weeks of pregnancy, the uterus undergoes spontaneous contractions which can be felt during a per abdominal examination. The uterus feels firmer at one moment and softer at another. These contractions are called Braxton contractions. They are irregular, infrequent, spasmodic and painless without any effect on the cervical changes like effacement or dilation.
Near term, these contractions become more frequent. It intensifies, and the patient may feel some discomfort. Ultimately, they merge with the painful uterine contractions of labour. On the other hand, in pre-term contractions, uterine contractions are more frequent, say more than 4 per hour. They are painful. The patient may feel a tightening of the uterus. Sometimes a patient may see a constant low back ache or some sort of lower abdominal pain or a pelvic pressure. These preterm contractions are always associated with cervical changes like effacement and also the dilation, which need immediate attention.
