Can IVF Help After Abdominal TB? | पेट की TB से Infertility हो तो क्या करें? | Dr Supriya Puranik

Hello friends, मैं Dr Supriya Puranik, IVF Consultant और Obstetrician-Gynaecologist. आज मैं आपको एक ऐसे केस के बारे में बताने जा रही हूं, जिसमें सारे reports normal थे, फिर भी pregnancy नहीं हो रही थी ;और आखिर में diagnosis मिला abdominal tuberculosis का। जब ये patient मेरे पास आई, तो शुरू में तो कुछ समझ नहीं आया कि problem है कहाँ। सब कुछ superficially normal था। लेकिन जब हमने depth में जाकर evaluation किया ;तो एक hidden reason सामने आया, जिसे अक्सर नजरअंदाज़ कर दिया जाता है। ये patient 2022 में मेरे पास आई थीं। उस समय उनकी उम्र थी 34 साल। उनके periods regular थे दोनों fallopian tubes open थीं उनके पति की semen analysis भी normal थी ;sperm count, motility, morphology, और यहां तक कि DNA fragmentation तक सब ठीक था फिर भी दो साल से pregnancy नहीं हो रही थी। IUI भी fail हो चुका था हमने पहले 3 cycles of IUI कराए ;लेकिन वो भी unsuccessful रहे। Diagnosis यही आया: Unexplained infertility. लेकिन unexplained का मतलब होता है ;"अब तक जो देखा, उसमें कोई वजह नहीं दिखी", न कि "वजह नहीं है"। इसलिए मैंने सलाह दी diagnostic hystero-laparoscopy कराने की ;और वहीं हमें पता चला कि उन्हें abdominal tuberculosis (TB) है। ये एक ऐसी condition है जिसमें uterus या ovaries को infection नहीं दिखता, लेकिन abdominal cavity में silent TB infection active होता है। TB का impact fertility पर कैसे होता है?
