Fresh embryo transfer means transferring the embryos into the uterus in the same cycle in which the eggs were retrieved and fertilised. It can be done either 3 days after the retrieval if there is the presence of cleavage-stage embryos, or it can be done 5 days after the egg retrieval if it is blastocyst embryos. So usually, after the egg pickup, the clinician does a scan to check for the endometrial thickness and runs a few blood tests to check for the hormonal levels. If the hormone levels are within normal limits and the endometrial thickness is good, they go ahead with doing a fresh embryo transfer. But in cases where the endometrial lining is not good, it is very thin, or the hormone levels are abnormal, or there is a risk of ovarian hyperstimulation syndrome, then they defer doing a fresh embryo transfer and plan for a frozen ET transfer.
