Ten4Ten: Because where you live shouldn't determine if you live

Ten4Ten is an ambitious, nationally scalable, community-led program aimed at transforming cardiovascular health in Tiny Towns, which are geographically defined communities with a population of less than 1,500. The initiative will demonstrate how place-based, tech-first health interventions can reduce preventable cardiovascular disease (CVD), improve health outcomes, and create long-term systemic change.
