Lung cancer is now the most common cancer among males in India, and it is one of the most common cancers throughout the world in males and females. So why I'm talking to you today is because awareness saves lives, and with awareness, if we can lead to early detection, we can cure the patient. So what are the risk factors of lung cancer? The most common risk factor for lung cancer is smoking, be it cigarette smoking, bidi smoking, hookah, e-cigarettes, or smoking along with passive smoking. That is, if someone else is smoking and you're standing nearby, it can also lead to lung cancer. Apart from smoke, other factors could be pollution, smoke from chulhas, asbestos, and certain other chemicals. Exposure to these factors can also lead to lung cancer. But nowadays, we are seeing that many patients are non-smokers. So what leads to that? It could be because of pollution and some genetic factors. So how do we know that something is wrong? Some of the warning signs or symptoms are a cough that persists for 2 to 3 weeks, a cough in which blood is present, shortness of breath, or wheezing that continues for up to two to three weeks. There is sudden weight loss, anorexia, loss of appetite, and constant fatigue. So if you are having any such symptoms, please consult your physician or doctor so that the signs can be caught early and the cancer can be diagnosed at the earliest stage. So, how to diagnose lung cancer? First of all, you need to visit the nearest physician or a pulmonologist who will advise certain tests. Such tests could be a chest X-ray, CT scan, bronchoscopy, bronchoscopic-guided biopsy, or, sometimes, if a bronchoscopic-guided biopsy is not possible because of the location of the mass or lump, then the doctor will advise you to go for a CT-guided biopsy. After such tests, when the diagnosis is confirmed, we have to do molecular tests as well because the treatment has advanced so much that molecular testing definitely changes the course of the treatment for the patient. After such tests, when the diagnosis is done, you can consult your oncologist. For the staging workup, the doctor will advise you to go for a PET-CT scan and an MRI of the brain, plus certain other tests as well, depending upon the stage. If we talk about the treatment options, lung cancer treatment has advanced significantly. According to the stage, in the early stage, the treatment options could be surgical evaluation followed by surgery or radiation therapy. In the locally advanced stage, the patient could be advised to go for chemotherapy followed by radiation therapy or surgical therapy. In the metastatic stage, that is Stage Four, we have targeted therapies, immunotherapies, and chemotherapy. According to the genetic test of the patient, the therapies are decided. If we talk about what targeted therapies and immunotherapies are, targeted therapies specifically act upon certain mutations, which could come under your NGS or mutation testing reports. Immunotherapy enhances the immunity of one's body by enhancing the T-cells and B-cells and directing them against the cancer. Chemotherapy directly kills the cancer cells which are already present. With the latest advancements, therapy is being decided by the genetic makeup of the patient, particularly what mutations the patient is harbouring. With the latest advancements, the side effects are minimal. Targeted therapies are becoming more and more common according to the mutation.
