Current guidelines say you don’t need to check estradiol levels in menopause. Women with estradiol below 5-10 pg/mL have significantly lower bone density and higher fracture risk. Only women at or above 25 pg/mL showed balanced bone formation and resorption. BMD increases up to about 70 pg/mL, then plateaus. It doesn’t stop at bone. The ELITE trial showed early estradiol slowed atherosclerosis progression. Higher levels are linked to lower total cholesterol, lower LDL, lower fasting glucose, and higher HDL in early postmenopausal women. These metabolic benefits were not seen in women more than 10 years postmenopause. In plain terms: adequate estradiol levels protect your bones, your arteries, your lipids, and your blood sugar. And the window to get the most benefit is early. Here’s the problem. Transdermal absorption is variable. I checked my own levels while my symptoms were controlled and found my estradiol was running at about half of the bone-protective range. My hot flashes were gone. My bones were not protected. You can't feel bone loss. You can't feel early atherosclerosis. You can't feel insulin resistance creeping up. But you can measure estradiol. And the data says it matters. Ettinger et al. (1998).
